Nutrition Class for Pre-op Bariatric Surgery: Coordinated with Southern Surgical Associates, P.A.

Class Time: We will be offering classes on Thursdays twice a month. One in the morning and one in the afternoon. Please contact our
office for more details and registration. Arrive 30 minutes prior to the actual class time to weigh in and check in. Be prepared to stay 3-4
hours for the class.
Cost of the class is $85.00. Payment and registration must be received 1 week prior to your class date. This will insure a spot for you.

Please make sure to make out the check or money order payable to Nutrition at Best, Inc. If you signed up late for the class, you can drop
off the registration form and payment at the office.
We take Visa/Debit/Visa Flex Plan, Master and Discover Cards. You may call our office and submit payment over the phone, come by the
office and pay it in person or send the information with the registration form by mail. To make a transaction we need the name listed on the
card, 16 digit account number, expiration date and the 3 digit security code listed on the back of your card.
**Note: Card information will be shredded and destroyed after transaction is approved. We do not keep any credit information on file.
*If you have BCBS of NC as your primary insurance company (ex. YPPW,YPDW) you may be eligible for 6 free nutrition visits. It is your
responsibility to call BCBS at 1-800-218-5295 for verification.
If you have any type of BCBS for your primary insurance, you need to call the 1-800 number on the back of your card to see if they cover for
Dietitian visits. Your insurance will be filed but it is not a Guarantee of payment. If the claim is denied, you will be responsible for payment.
If you are eligible for nutrition visits, you must enroll and contact Jennifer Elias (Dietitian) at 252-757-0028 before submitting your payment
and verify coverage.
At the current time, no other insurance companies cover for the nutrition class and patients are responsible for payment.
**If you bring a family member to one of these classes they may have to wait in the lobby as we have only enough seats for our patients in
the class room.

Take a left at the stoplight from the parking lot. At the next stoplight (Arlington Blvd), take a right.
Take the 2nd left on to Emerald Place. We are the 1st building on the right.
If you pass the Railroad Crossing and K & W, you have gone to far.
If the class time is not conducive for you, you can call to see if a 1 on 1 consultation better suites you.
